*PLEASE HELP ME* How tomake a picture disc on Photoshop CS5 ?

I answered in the Pm you sent me, as well as the tumblr message  :creep:  I made the template completely by myself in photoshop looking at the blue jeans picture disc as reference. So i'm not gonna share it, but feel free to make your own
