I can't believe Mat Maitland has designed the covers (you might have seen his work from Goldfrapp!)
 

Then I checked his website and

I'm pretty sure it is because her hair is the exact same in this photo as in another picture I've seen and also her face doesn't look quite right in it

They photoshopped her face/hair from a GQ Awards candid pic. Idk where the body is from - maybe the candids from Chateau Marmont or the Ride premiere.
