There is conflicting information. Some of the people who worked on it said that the released version is the "original", but it's unclear if they mean with that. Until we get a leak of one of the 2013 mixes we won't know exactly how much changed. It seems the song used to be even more stripped back according to the blog post.

The vocal tone has to have a level of the room within it. Not a room mic, but even just in terms of how we're compressing, or where we're placing her, there has to be enough feeling of the space.

No, but as far as I understand it the version in A&W is close to the original Jimmy, they just added some ad libs and distortion effects. Lana said in an interview that she recorded a demo of American Whore in a hotel, sent it to Jack and he didn't look it at for half a year. Based on that it's not from the NFR era.
